(0:02) You will see that there is an example that has been done for you. (0:07) On this occasion only, the conversation relating to this will be played first. (0:16) Hello, this is Medical Help Direct.

How can I help you? (0:21) Oh, hello. I've got a problem with my back, and with it being the middle of the night, (0:26) I can't go to the doctor. And I wonder if you could give me some advice.

(0:29) Certainly. But first, can I have your telephone number, please? (0:34) It's 03786-439251. (0:43) The man's telephone number is 03786-439251.

So, 03786-439251 has been written in the space. (1:04) Now we shall begin. You should answer the questions as you listen, (1:10) because you are recording a second time.

Listen carefully, and answer questions one after the other. (1:23) Hello, how can I help you? (1:28) Oh, hello. I've got a problem with my back, and with it being the middle of the night, (1:32) I can't go to the doctor.

And I wonder if you can give me some advice. (1:36) Certainly. But first, can I have your telephone number, please? (1:42) It's 03786-439251.

(1:46) Okay. And if you could give me your first name, please? (1:50) Robert. (1:52) And your surname? (1:53) Rydyard.

That's R-I-D-Y-A-R-D. (1:59) And now your address, please? (2:01) It's 27 Station Road. (2:04) Okay.

(2:05) Moortown. And do you need the postcode? (2:08) Yes, please. (2:09) D-P-17-5-H-J.

(2:13) And your date of birth? (2:15) The 25th of February, 1975. (2:19) Fine. Now I just need to take details of your doctor.

(2:23) She's called Dr. Craven. (2:26) And the surgery? (2:27) It's Moortown Health Centre. (2:29) Thanks.

Now, can you tell me a bit more about what the problem is? (2:33) Well, like I said, it's my back. (2:35) Okay. Can you tell me which part of the back? (2:38) Lower.

(2:39) And have you had any problems with your back before? (2:43) Yes. I had a bit of trouble last year, in June, I think. Or was it July? (2:47) No, we went on holiday at the beginning of July, and it had cleared up by then.

(2:53) And when did it start this time? Has it just come on? (2:56) No, I've had it, let's see, since Monday, so that's three days now. (3:02) But it wasn't too bad at first. I mean, I could just about get around, though.

(3:06) I couldn't drive the car or anything. (3:09) Then this afternoon, the pain suddenly got so bad that I went to bed. (3:13) I've been to bed since, but that hasn't helped.

(3:16) And do you have any pain elsewhere? In your stomach or abdomen, for example? (3:21) No. (3:25) Before you hear the rest of the conversation, (3:28) you have some time to look at questions seven to ten on page two. (3:54) Now listen and answer questions seven to ten.

(4:01) Okay. Now, have you any idea what caused the problem? (4:06) Well, I was tidying up my office, (4:08) and I lifted a box up from the floor that was heavier than I thought. (4:13) And I think that's what did it.

(4:14) Yes, it's easily done, isn't it? (4:16) So you're in bed now? (4:18) Yes. (4:19) Have you tried applying heat to the back? (4:21) Yes, I've got a hot water bottle. That's usually the first thing I try.

(4:25) And have you taken any painkillers? (4:27) Yes. (4:28) And other than that, you feel quite well? (4:31) I do, yes. (4:31) Right.

Now, I can't diagnose or prescribe any medication, but I can offer you some advice. (4:39) One thing you could do now is to see if ice makes any difference. (4:43) That sometimes helps.

(4:44) But don't put it directly onto your skin. (4:47) Wrap it up in a towel or something. (4:49) And don't leave it on for longer than 20 minutes.

(4:52) Oh, right. I haven't tried that before. (4:54) It can be useful sometimes.

(4:57) Now, have you seen your doctor about this? (4:59) No, I just thought, well, everyone has back pain. (5:03) But I've never had it as bad as this before. (5:05) I think you should go and see your doctor tomorrow.

(5:08) And she may be able to give you something to help. (5:10) OK, I'll do that. (5:12) I'm actually feeling a bit better now.

(5:14) Good. And you might get some advice from her about exercise. (5:18) That's often the best way of dealing with this sort of problem.

(5:20) OK, I'll ask her. (5:22) Right. Now, if you're worried about anything before tomorrow, (5:26) you can always phone back.

(5:28) Our lines are open 24 hours. (5:30) Thanks very much. (5:35) That is the end of section one.

(5:37) You now have half a minute to check your answers.
